  • Police reports and body camera footage can be obtained by contacting the police agency involved; ask to be transferred to the records department, and make a formal request for all records (including video) pertaining to the incident date.

  • Medical records can be obtained by contacting the hospital/provider involved; ask to be transferred to records, and make a formal request for all records related to the treatment from the original incident date to the present.

  • We will also need a written account of the incident, from beginning to end, from your perspective, to compare to the police reports.
